Medical Staff Privileges

In addition to the Maryland Hospital Credentialing Application, available via the link below, Baltimore Washington Medical Center requires additional information and forms to be completed.

Please contact the Medical Staff Services Department for more information on obtaining medical staff privileges for both physicians and licensed independent practitioners by e-mailing medicalstaffservices@bwmc.umms.org. Please provide your full name, address, email address, phone number and specialty. You may also fax your request to (410) 595-1992, or request an application by phone at (410) 787-4525.
